Go to Autozone and rent ( borrow because it's free for DIY) the Scan Tool and check for Code and Error Message. If something relates to Gas tank pressure sensor then just release gas cap and tight again until you hear three click sound.
Post the Code if you can then i can explain the problems.

I don't think your van have any problem, whenever you change the engine oil, you need to (reset) do clear the 5000 miles count.
Try the infor below. hope it help.

This light flashes for 12 seconds after start up when the odometer "thinks" it is time for an oil change. Easy to clear:a) select ODO; 2) turn engine off; 3) hold reset button and turn ignition on; 4) hold for five seconds -odometer reads 0000000 and light goes out. I guess one should read the manual more than the first time out the door ...

at428,
You're wrong. Toyota LED indicates "Maintenance Req" that means for Schedule Maintennance (like oil change, filter etc.) NOT "Check Engine" light on. Check Engine light on means something wrong with the engine. Do what Ty1Ong suggestion.
Your suggestion is for Maintenance Req.

Thank you for correction, you are right, I go out and check my dad Van sure it do have two lights, one for Engine light and one for Maint. light. This happened my dad Van also, it happen somewhere around 10000 miles, because after the service they forget to reset the Maint. light --
again, thank you for correction. Merry Christmas and Happy New Year

If U have the "Check Engine" light on, it receive signal from some sensor that something does not function right. This can lead to Emission Test fail.
For Toyota, I've seen this happens to few models (Sienna, Camry, Celica) from 10-60K miles:
1) Inital problem is the gas cap not tight (or bad gas cap)
2) Injectors need to be cleaned
3) Major tuneup needed
4) Catalyst converter is faulty
For your case (12K miles), (1) or (2) might apply.
Just like ty1ong suggests, get the error code to find out exactly what it is.

I get this msg "system too lean". Buying Fuel Injector Cleaner can make the light disappear .... for month :D
What is "fuel is too lean" mean in the diagnostic test?
A: Fuel Too Lean is possibly one of the most difficult computer codes to diagnose. Why is that? There are a couple of things that you need to understand first, before you understand what too lean means. An engine to run correctly needs a good balance of air and fuel. This balance is called the air /fuel ratio. A correct balance of air and fuel is generally at a ratio of 14.7 pounds of air to one 1 pound of fuel. If you change either number of that ratio, is will have an effect on the engine's performance. The computer in your vehicle is always working to maintain the correct air/fuel ratio. However, as time goes on the engine wears, fuel injectors become restricted, vacuum leaks happen and sensors may become worn and/or lie to the computer. After all, it is a rough environment under the hood of your vehicle. The engineers designed the program in your vehicle's computer to make adjustments to compensate for changes that occur with time. This ability of the computer is called adaptive learn. But, there is a limit to how much the computer can adapt, all the while, maintaining the ideal air/fuel ratio. If the computer determines that it cannot adapt any more to provide enough fuel for the amount of air entering the engine it will turn on the check engine light and report a code indicating that the system is too lean. Too much air entering the engine for the amount of fuel available. There is also a code for the opposite condition of the engine receiving too much fuel, system too rich. In either case, the code does not indicate a broken sensor, all it means is that the computer cannot maintain the correct air/fuel ratio and it wants help in fixing the problem. I've seen many parts replaced because of this code that did not fix the problem. You'll need a technician that understands and has the necessary diagnostic tools to provide the correct solution to a system too lean computer code.

I presume your van is still under waranty. Take it to the dealer to check. You got nothing to lose. If you let it runs like that for long, it could damage other part like the CAT (converter), ..., and it will be more expensive later.
Engine runs lean and rich is due to oxygen sensor functionality. If you hook a scanner and watch the O2 sensor data, it should toggle back and forth. If the engine runs lean or rich, it could be the O2 sensor out.

Just stop by your dealer auto repairshop and change new oil and .... Yes, most new Toyotas were designed with that reminder when they reaches manufacture preset mileage. Anyway, ask if the dealer charge a fee to turn that light off. My friend was charged $25 dollar for just turning that shit light off. You can do that yourself.
Under warranty you should not pay at all.
